Description of original award (Fiscal Year 2023, $750,000)
PathStone Corporation, a multi-state 501(c)(3) non-profit requests $750,000 to develop and implement a comprehensive community-based adult reentry program to serve a minimum of 75 individuals incarcerated in the Luzerne County Correctional Facility. Individuals to be served by the transition to Tomorrow (T2T) program will be determined, utilizing the StrongR criminogenic risk assessment as having moderate to high recidivism risk. Enrolled individuals will be eligible for release within 90 days and will be residents of Luzerne County, Pennsylvania.
PathStone knows from experience that career training and employment sets the stage for a participants' future achievement/success. This combined with utilization of evidence based cognitive behavioral therapy, family reunification, and mentoring along with well-developed individualized case management, including program supportive services or referrals, are the keystones to the success of the T2T program. Services will begin pre-release and continue post release.
Luzerne County has a 3-year overall recidivism rates of 63.7%. Our program goals include: a one-year recidivism rate of less than 22%; employment placement rate of 60%; and employment retention rate of 60%. PathStone adult reentry staff is experienced, well-versed, and trained and/or certified in a variety of evidence-based programs inclusive of: cognitive behavioral therapy “Thinking for a Change; Offender Workforce Development Specialist; Reentry Employment Specialist; Motivational
Interviewing; Mental Health First Aid; Crisis Intervention Training; Risk Assessment administration/interpretation. Additional trainings will be provided to staff during the program planning period and program operation, including training for the StrongR risk assessment tool.
Components of PathStone's approach to promote equity will include active participation in community events held by these stakeholders, as well as advocacy through providing opportunity for upward mobility for program participants form these populations. Feedback from these interactions will be solicited from all parties; and PathStone is a member of both county reentry task forces and
will continue to work in collaboration with other task force members to coordinate reentry services and develop new strategies.
