The proposed project co-partnered between the Albany County Department of Mental Health and the Albany County Probation Department, aims to reduce recidivism and address criminogenic needs in Albany County, New York. The project will concentrate on enhancing support for individuals released from incarceration, particularly those referred from the Probation Department.
The project's core objective is to develop a robust “jail re-entry” case management model for individuals with serious and pervasive mental illness. This entails fostering relationships and improving connections to community services, post-release. The project aims to assist approximately 120 individuals over a three-year period.
Key project activities encompass optimizing support for individuals exiting jail, establishing resource connections, providing peer support, and addressing social determinants of health through case management. There are no planned conference activities for the recommended award.
