Description of original award (Fiscal Year 2024, $1,000,000)

The Colorado Department of Corrections Division of Adult Parole proposes to develop a Pay for Performance Program that coordinates recovery residences for individuals with serious mental illness for the first 6-9 months of reentry and for re-stabilization of individuals before a crisis occurs. This work will also include providing behavioral health services within the residences to support these vulnerable individuals and provide a step-down process to successfully transition individuals from corrections, to community, and into independent living. The Division of Adult Parole will collaborate with several existing partners to provide comprehensive case management and wrap-around services. Pay for performance partners will be selected through a request for information process and will be expected to provide either recovery housing and/or peer support to reduce the number of individuals who abscond, need hospitalization, receive jail time, or revocation and improve the overall reentry experience.
