Description of original award (Fiscal Year 2024, $1,000,000)

The County of Putnam proposes to implement the “Reaching Rural: Rural Roads to Recovery Program.” The purpose is to develop a collaborative, research based, and multi-care level community-based program for individuals affected by substance use disorder (SUD) across a three-county catchment area including Marshall, Putnam, and Stark located within Illinois. Project activities include establishing a network comprised of project leadership, county lead contacts, and various organizations and individuals within the community; developing efforts within an established rural community wraparound center and a future wraparound center providing access to substance use resources; establishing medication assistance treatment services; naloxone distribution; youth prevention and educational for substance use training; increasing mental health and treatment services; and provide increased access to SMART Recovery and Narcotics Anonymous meetings. Expected outcomes include the development of a multidisciplinary team within the network to continue sustainability efforts and continuous growth and improvement; increased collaboration between the judicial, treatment, and community groups; reduction in recidivism, and increased multi-disciplinary programming to provide necessary skills. Subrecipients include Maitri Path to Wellness, the Henry and Stark County Health Department, and The Society of Growth Inc.
