Supporting Continuity of Care throughout Justice Involvement: Effective and Safe Transitions to Community-based Treatment

The transition from incarceration to the community is a critical time in terms of treatment continuity and ensuring people with substance use disorders (SUDs) are able to access important necessities of housing, transportation, and other basics of survival to support their treatment engagement. People experiencing SUDs are at high risk of overdose and overdose deaths during the two weeks following release from incarceration.

Part of the Supporting Continuity of Care throughout Justice Involvement series, this webinar will present programs and processes that jails should have in place to ensure that people are able to access and/or continue substance use treatment, link with peer support or other social supports, and connect with resources that enable them to make progress toward safe and stable housing.

Learning Objectives:

  • Describe effective reentry planning processes for people experiencing SUDs.
  • List two ways to ensure people can continue medications without interruption following release from jail.
  • Discuss key partners for ensuring smooth transitions for people with SUDs reentering the community.
