Description of original award (Fiscal Year 2024, $1,300,000)

The Fond du Lac County Health Department aims to implement the Fond du Lac County Bridges to Recovery Program. Fond du Lac County serves suburban and rural areas served. The purpose is to expand the current pathways of care in the Fond du Lac County jail. Project activities include increasing peer support services, expanding services to include mental health counseling with substance use counseling; incorporating the three forms of medications for addiction treatment for compliance with the Americans with Disabilities Act (ADA), reducing stigma for treatment, and increasing post release services to prevent relapse and recidivism. Expected outcomes include increased participation; additional provision of evidence-based services, reduction in substance use; reduction in recidivism; and educational training for correctional staff. Residents of the county and the surrounding areas, who are involved in the criminal justice system at the Fond du Lac County jail, are the intended beneficiaries of the project, a typically underserved community.
