Nominator (Recommending Official) Information
This section will be used to capture information about the agency head  who is nominating/recommending an individual for the Congressional Badge of Bravery.

Name
If the nomination form is being filled out on the behalf of the recommending official/nominator, the name of the recommending official/nominator should be entered here (not the name of the individual filling out the nomination form).

Other Title(s)
If the recommending official/nominator's  title is not listed in the "Occupational Title" drop down menu, please enter their title here.

Name of Appointing Authority
This is the name of the agency where the recommending official/nominator serves as agency head.

Nominee Information
This section will be used to capture details about the individual being nominated to receive the Congressional Badge of Bravery.

Other Title(s)
If the nominee's title is not listed in the "Occupational Title" drop down menu, please enter their title here.

Grade/Rank
This is the nominee's official rank or skill-level title at the time of the act of bravery.

Field Office Address
This is the address of the agency where the nominee served at the time of the act of bravery.

Years of Service
The number of years of service by the nominee as of the date when the act of bravery was performed. If service time is less than 1 year, you may enter the length of service in months (i.e. 6 months).

Event Information
This section will detail the act of bravery and when it occurred; including, the date(s), location and a description of the act.

Date Range
Enter the start and end date(s) of when the act of bravery occurred. If the act of bravery occurred during the span of one day, the start and end dates will be the same.

City, County, Township
This is the name of the city, county or township where the act of bravery occurred.

Summary of Act of Bravery
This written narrative should describe the circumstances of the event when the nominee performed the act of bravery. An explanation of how the circumstances meet the criteria of the Congressional Badge of Bravery should be included within the summary. Please note that the Summary of Events can be no more than 2 pages in length and has a limit of 7,500 characters.

Witness Information
Although not required, this section can be used to supply contact information for the individual(s) that witnessed the act of bravery. If you choose to include witness information, please be sure to include the witness' name, address, and telephone number.

Title
If applicable, enter the witness' official title.

If the witness' title is not listed in the "Occupational Title" drop down menu, please enter their title here.

Add another witness
You may include more than one witness to the act of bravery. Please know that there is no limit to the number of witnesses entered, and, the nominator can also be listed as a witness.

Supporting documents can be submitted for inclusion with the nomination. Files can be submitted in the following formats but should not exceed 5MB each:

Supporting documents may include items such as, official reports, video footage/coverage, new articles, photos, community/state awards presented to the nominee in recognition of the act of bravery, etc.

Supporting Documents
Please do not attach video footage to the nomination form. Instead, you can send any video footage to Congressional Badge of Bravery program staff at:
